- Plans must reply to help requests within one business day.
- For urgent requests, plans must give a list of doctors accepting new patients in two days; five days for non‑urgent.
- Online directories must be free, searchable, and need no login.
- Directories must be updated at least weekly and include many provider details.
- Printed copies must be mailed within five business days after request.
- Directories must disclose interpreter services and disability access rights.
- Violating these rules is a crime.
