- The summary must include definitions, coverage details, cost sharing, exceptions, renewability, examples, and essential coverage status.
- It must list contact info, web address, network provider list, formulary details, and a glossary.
- The summary must be given to applicants within 7 business days, to insureds within 30 days of renewal, and on request within 7 days.
- Insurers must give detailed provider network and credential info when asked.
- Policy documents must use uniform font size of at least 10‑point and clear formatting.
- Information can be provided electronically if allowed.
