- It applies the annual limitation from 42 U.S.C. 18022(c)(1) to all health plans and pharmacy benefit managers.
- Insurers must include any cost‑sharing amounts paid by the enrollee or on their behalf when calculating contributions.
- The rules apply to plans entered into, amended, extended, or renewed on or after Jan. 1, 2027.
- Insurers may not set plan terms based on the amount of financial assistance a prescription drug receives.
